Patient Forms
Patient Forms are available electronically and will be sent to you via text or email prior to your first visit. You can save some time at your appointment by completing the Patient Forms before your office visit. If your are unable to complete forms online or require assistance please plan to arrive 15 minutes early to your scheduled appointment time.
What To Expect
During your first visit you will get to know your dentist while discussing your medical history, any on-going concerns you may have and work together to plan your dental health goals. You will also receive a thorough oral exam from your provider at this time.
We recommend that you review our Insurance and billing practices and take some time to review our staff page to familiarize yourself with our office. Being well prepared for your appointment will ensure that the doctor has all of the necessary information to provide you with the best possible care. We look forward to welcoming you to our practice.
We are happy to provide off street parking during your appointment time in the lot located directly behind our office. An ADA accessible entrance is accessible from the parking lot at he rear of the building.
